Why do I need an exchange card during pregnancy?
No one will argue that pregnancy is a very important period in a woman’s life. Like any other serious matter, pregnancy should be accompanied by a documentary, and for this there is an exchange card.
The inclusion in one document of all the important data on pregnancy, childbirth and the newborn – that’s what an exchange card is for.
It is given to all pregnant women in the women’s consultation, where they fill in the necessary data on the condition of the mother and the future baby. This information will later be useful in the maternity hospital and the children’s polyclinic.
Correctly and in detail-filled exchange card pregnant allow any physician (not just the one who is pregnant in consultation) be aware of the peculiarities of carrying a child, and after delivery will be a source of information for the hospital neonatologist and pediatrician children’s clinic.
This is important for any pregnant woman a document issued by the district doctor from the women’s consultation at a period of 22-23 weeks. It is in this interval that the last diagnostic procedures are carried out for the presence of malformations of the fetus, and in the next weeks the termination of pregnancy will for some reason be considered an artificial birth.
In some women’s consultations, a pregnancy card is issued after 28 weeks. It also happens that the exchange card is issued earlier, for example, when the expectant mother enters the department of pregnancy pathologies earlier than 22 weeks.
It is also necessary to know when to sign the exchange card at the selected hospital. Usually this is addressed to the head physician for a period of 34-36 weeks.
This document consists of three main sections, respectively filled in the women’s consultation and the maternity hospital (about the parturient and the newborn). Let’s consider in more detail each section.
1. Information to be filled in women’s counseling, contain data on previous pregnancies and childbirth, as well as on the features of the current bearing of the child. Also in the exchange card at each visit of the future mother to the consultation the doctor records the result of the examination and examinations. All tests given by a pregnant woman should be reflected in this part of the exchange card.
This section will be very important when a pregnant woman enters the maternity ward, and if a woman does not pass the required examinations, she goes to the specialized obstetric ward for labor with infections.
2. Information about a woman in childbirth, filled in the maternity hospital. The obstetrician fills this section in the hospital, describing the course of labor, the features of the postpartum period and the condition of the parturient at the time of discharge (if necessary, indicate the features of observation).
